When to hire your first admin support

Most allied health founders hire admin support twelve months later than they should. The trade-offs of waiting versus moving, and the signals that tell you it is time.

If you are running a clinical caseload and doing your own admin, you are operating two practices simultaneously. The clinical one is what you trained for; the admin one is what you taught yourself. The reason most allied health founders delay hiring admin support is not that they cannot afford it. It is that they cannot picture handing it over.

Five signals it is time

  • You are doing admin past 7pm two or more nights a week.
  • Referrals are sitting in your inbox for more than 48 hours.
  • Invoices are going out late and you are chasing payment instead of seeing clients.
  • You have stopped responding to non-urgent parent emails because they have piled up.
  • You can describe a clinical decision you delayed because of admin overload.

What to hire

Most first admin hires in allied health are part-time — 12 to 20 hours a week. The role is referral management, scheduling support, invoicing/billing, parent communication overflow, and the one-off operational projects that never quite get done. You do not need an industry specialist; you need someone organised, calm, and adaptable.

The financial math is not what you think

If a part-time admin frees up four hours of your clinical time a week, and your billable rate is well above their hourly cost, the math is positive within the first month. Most founders run this calculation and still delay because the abstract risk of handing over feels heavier than the concrete cost of doing it themselves.

Hire ahead of the breaking point

Hiring during a crisis goes badly. You do not have the bandwidth to onboard them well. Hire when you can see the signals listed above for three consecutive months — not when you have already missed a client communication that mattered.
